Terms of Service

Welcome to Evolve Therapy & Wellness’s website and telehealth platform (the “Site” and “Services”). By accessing or using our Site, scheduling appointments, or engaging in therapy services through our telehealth platform, you agree to these Terms of Service (the “Terms”). Please read them carefully.

If you do not agree with these Terms, please do not use our Site or Services.

1. Services Provided

Evolve Therapy & Wellness provides professional mental health counseling and psychotherapy services to clients in the state of Connecticut through secure telehealth technology. Therapy services are delivered by licensed clinicians authorized to practice in Connecticut.

Telehealth is not appropriate for all mental health needs. If you are experiencing a crisis or emergency, call 911 or go to your nearest emergency room.

2. Eligibility

By using our Services, you represent that:

  • You are at least 18 years old, or are the parent or legal guardian of a minor receiving therapy services.
  • You reside in Connecticut, where our clinicians are licensed to practice.
  • You agree to provide accurate, complete, and current information.

3. Not for Emergencies

Our Services are not intended for medical emergencies or urgent situations. If you believe you are in immediate danger of harming yourself or others, please call 911 or go to the nearest emergency room.

In the U.S., you can also dial or text 988 for the Suicide and Crisis Lifeline.

4. Payment, Insurance & Fees

Payment

Payment is due at the time of your session unless other arrangements are made. We accept:

  • Payment Plans: Available upon request for out-of-pocket clients.
  • Insurances Accepted: Aetna, Cigna, UnitedHealthcare, Anthem/BCBS, and Oxford.
    Please note we do not accept HUSKY, Medicaid, or Medicare.
  • Sliding Scale: Reduced fee options may be available on a limited basis. Please inquire during your initial consultation.

5. Cancellation & Rescheduling

Cancellation Policy

We require at least 24 hours’ advance notice if you need to cancel an appointment. Cancellations made with less than 24 hours’ notice, or missed appointments (no-shows), will be charged the full session fee.

Rescheduling Policy

Clients may reschedule appointments up to 24 hours in advance through our secure client portal.

6. Privacy & Confidentiality

Your use of our Site and Services is subject to our Privacy Policy and our HIPAA Notice of Privacy Practices. These documents explain how we protect, use, and disclose your personal and health information. By using our Services, you consent to these practices.

7. Technology & Telehealth Risks

Telehealth depends on internet and technology that may be interrupted or encounter failures. While we use encrypted, HIPAA-compliant platforms, you understand:

  • Electronic communication carries confidentiality risks outside our control.
  • You are responsible for using a secure, private location and device for sessions.
  • You will inform us if your location changes or if technical difficulties disrupt services.

8. Intellectual Property

All content on our Site, including logos, graphics, text, and software, is owned by or licensed to Evolve Therapy & Wellness and is protected by intellectual property laws. You may not copy, distribute, or exploit any content without our prior written permission.

9. Limitation of Liability

To the fullest extent allowed by Connecticut law, Evolve Therapy & Wellness and its clinicians will not be liable for any indirect, incidental, consequential, or special damages arising from your use of the Services, or from interruptions in service. Our maximum liability will not exceed the amount you paid for services in the prior three months.

10. Changes to These Terms

We may update these Terms occasionally. Any changes will be posted on our Site with the updated effective date. Your continued use of our Services after changes means you accept the updated Terms.

11. Governing Law

These Terms and any disputes arising under them are governed by the laws of the State of Connecticut. Any legal action shall be brought in the state or federal courts located in Connecticut.
